Home Assessments for Safety & Independence

The home modification process begins with a face-to-face appointment where we conduct a thorough and comprehensive assessment of the participant's home environment. During this session, we evaluate the participant's functional capacity and limitations, discuss their full medical history and identify any hazards within the home.

Key areas of focus include assessing the bathroom and toilet environment, as well as access to the home. This includes a functional assessment of the participant accessing and transferring within these regions. By meticulously examining these aspects, we can identify potential risks and implement effective solutions to enhance the participants's quality of life and autonomy within their home.

Tailored Solutions for Enhanced Mobility

Based on the initial assessment, we identify specific modifications that can significantly improve the participants's safety and independence. Examples of these modifications include the installation of grab rails within the bathroom, shower and toilet environment, considering non-slip tile treatment within the shower alcove, and installing access ramps to the home to ensure safe and independent access.

Additionally, we consider modifications to improve access to both the front door and the backyard. These tailored solutions are designed to address the unique needs of each participant, ensuring they can navigate their home environment with greater ease and confidence.

Planning & Coordination With Builders

Once the necessary modifications are identified, we take the next step by creating detailed designs and diagrams. These plans are then sent to a builder to obtain a quote for the proposed modifications. This collaborative approach ensures the modifications are both practical and professionally executed.

After receiving the builder's quote, we submit a request to the NDIS for funding approval. Our goal is to ensure the entire process, from planning to implementation, is seamless and efficient, allowing participants to benefit from the home modifications as soon as possible.

Implementation & Ongoing Evaluation

After the NDIS approves the funding and the builder completes the installation of the recommended home modifications, we conduct a follow-up visit to ensure everything meets the participant's needs and expectations. As occupational therapists, we are committed to making sure the participant is satisfied with the modifications and that any necessary adjustments are made promptly.

We conclude the process with a final evaluation, revisiting the initial comprehensive assessment to review the participants's limitations, safety, fall history, and access to all areas of the home. Our goal is to enhance the autonomy, safety, independence and overall living conditions so the participant can enjoy a more comfortable living experience.
